Once a height difference exceeds 5/8 in (0.625 in), it's past what a standard T-molding or seam binder is built to cover — the strip either sits proud, leaves a visible gap, or has nothing to grip. At that point the fix moves to a ramp-profile transition (rated to about 1-1/4 in), self-leveling underlayment, or a doorway threshold that may need to meet the ADA's 1/2 in maximum.
Off-the-shelf transition strips — T-moldings, seam binders, and most reducer profiles — are extruded around a specific range of height differences, and 5/8 in (0.625 in) sits near the top of what the common retail profiles are designed to hide cleanly. Below that number, the strip's lip or reveal has enough material to bridge the gap without flexing, gapping, or exposing raw subfloor edges.
Past 5/8 in, the geometry stops working. The strip either has to sit up too high off one side (creating its own trip edge), the retaining track has nothing solid to clamp into, or the visible reveal gets wide enough that it reads as a repair rather than a finished transition. Manufacturers rate their reducer strips up to about 3/4 in (0.75 in) for a moderate step-down, and dedicated ramp-profile strips extend further, to roughly 1-1/4 in (1.25 in) — but both of those are specialty products, not the strip pulled off a big-box shelf.

The molding flexes every time it's stepped on, which fatigues the adhesive or fastening track, and loosens the strip within a season or two — the strip pulling loose at a tile-to-carpet or tile-to-hardwood joint is one of the most common callback complaints installers see.
At doorways and accessible routes, an oversized transition also becomes a code problem, not just a maintenance one. The ADA caps threshold height at 1/2 in (0.5 in) for accessible doorways, and any unbeveled vertical rise above 1/4 in (0.25 in) has to be treated as a step, not a threshold. A 5/8 in-plus gap left as a sharp edge instead of a ramped or leveled transition is both a fall hazard and, in a commercial or rental setting, a liability exposure.
In wet areas the stakes go up again — a gap that's too wide for its strip lets water track sideways under the flooring edge instead of shedding across the top of it, which is exactly the setup that leads to lifted vinyl planks or a soft, spongy subfloor at the transition line months later.

First is location: a threshold on an accessible route or public egress path is bound by the ADA's 1/2 in limit and the 1:2 slope rule for any bevel (a value of 2 — two units of horizontal run for every one unit of rise) under Section 303.3, while a transition tucked inside a private closet or between two bedrooms has no code ceiling at all, only a safety and looks judgment.
Second is flooring type. Floating floors like LVP and laminate can ride over a flexible reducer because the plank itself has some give at the edge; rigid tile can't flex, so a large gap next to tile almost always has to be leveled in the substrate rather than bridged in the molding. Third is whether the difference is a step-down or a lip — a strip rated for a straight vertical rise won't necessarily perform the same on a sloped or uneven interface, which is why ramp-profile strips (rated to 1.25 in) exist as a separate category from flat T-moldings.

Self-leveling underlayment is formulated to be poured in a single lift anywhere from 1/8 in (0.125 in) up to about 1-1/2 in (1.5 in), which covers the vast majority of transition problems that start life as a 5/8 in-plus discrepancy — pour it against a temporary dam at the doorway, let it feather to zero at the edges, and the finish flooring goes down flat with no oversized strip needed.
For gaps too deep for one SLU pour, or where ripping out the subfloor isn't practical, a ramp-profile transition strip rated to 1.25 in is the fallback, provided the location isn't on an ADA-governed route where the 0.5 in cap applies. Self-leveling underlayment poured in a single lift of roughly 1/8 in to 1-1/2 in (0.125 in to 1.5 in) is the usual non-demolition fix, feathered down to zero at the doorway so the new flooring sits flush without relying on an oversized transition strip.
