Tile itself, being noncombustible, can be run with 0 inch clearance right up to the edge of a firebox opening. What can't get closer than 6 inches is any combustible material behind or around it — furring strips, OSB, wood trim — and from 6 to 12 inches out, that combustible material can only project 1/8 inch for every inch it sits away from the opening, per NFPA 211.
NFPA 211 is the standard most local codes lean on for fireplace clearances, and it draws its lines from the edge of the firebox opening, not from the mantel or the wall corner. Inside 6 inches of that edge, no combustible material is allowed at all. From 6 to 12 inches out, combustible material is allowed but can't project toward the opening by more than 1/8 inch for every 1 inch of distance — so the farther out you go within that band, the more a piece of trim is permitted to stick out. Tile, porcelain, stone, and brick are all classified as noncombustible, which is why installers can run the field right up against the firebox frame with 0 inch clearance. The rule was never really about the tile face — it's about what's underneath it. If the substrate behind that last course of tile is combustible plywood or untreated wood furring, the 6 inch clearance still applies to that substrate even though the finish material sitting on top of it is fire-safe.
That distinction is why the same question keeps showing up after reading about heat-rated backer board: the board and the tile are judged separately. A cement or heat-rated backer board satisfies the noncombustible requirement for the 0-6 inch zone; a standard drywall or OSB substrate does not, no matter what gets tiled over it.

Local code adoption is the second variable. Most jurisdictions adopt NFPA 211 with few changes, but some AHJs amend the clearance figures or require an inspection sign-off before the surround gets covered. That inspection matters for insurance too — a fireplace finished outside the adopted clearance can complicate a claim later even if nothing ever catches fire.
Inside the 6-12 inch band, material selection does the rest of the work: cement backer board rather than drywall or OSB, and a mortar and grout rated to tolerate sustained radiant heat rather than a standard modified thinset. A standard latex-modified thinset installed inside the 6-12 inch zone takes repeated cycles of radiant heat every time the fireplace runs, and over a few burning seasons the polymer modifiers in that mortar can break down faster than they would on a cooler wall. The result shows up as hollow-sounding tile, tenting, or tiles that pop loose near the opening years after a clean install — a problem that traces back to mortar choice, not workmanship.
The second failure is a straight code violation: a wood mantel, painted MDF surround, or combustible trim piece sitting inside the 6 inch zone, or projecting further into the 6-12 inch band than the 1/8-inch-per-inch rule allows. Beyond failing inspection, that trim is sitting closer to sustained heat than it's rated for, which is a real scorch and ignition risk during long burns, not just a paperwork issue.
The third is the expensive one: a failed final inspection after the surround is already tiled and grouted means pulling permits again, and often tearing out finished work to correct a clearance that should have been measured before the first tile was set.
