Large-format tile (anything with an edge over 15 in) is more vulnerable because there are fewer grout joints per square foot to share the load, so any single missing movement joint carries more consequence across a bigger span.

Timing matters too. Tenting that shows up within days of installation usually means a movement joint was never cut in, or the mortar hadn't cured before grout locked the field rigid. Tenting that appears months or years later, particularly after a season change, a slab pour nearby, or a wet event, points to substrate-driven moisture movement rather than an installation-day mistake.
Run a hand along the ridge and check the grout on either side. Cracked or crumbled grout concentrated at that one joint, combined with lippage that only appears at that line, confirms a compression failure at a specific joint. That means cutting out the tented tiles plus enough of the surrounding field to install a real movement joint at the correct location: remove tile and mortar back to a clean edge, cut the substrate joint if one is called for, and install backer rod with a flexible sealant (or a manufacturer-rated soft joint) at the width and depth the tile and sealant maker specify. Spot-regrouting the ridge without cutting a joint is not a repair — it just re-locks the field.
If the diagnosis points to slab curling or moisture-driven movement, the substrate itself needs attention before new tile goes back down — that can mean an uncoupling or crack-isolation membrane under the patched area, or letting the slab finish drying and confirming with a moisture test rather than guessing.
Replace tile across the full width of the affected joint rather than trying to save the cut edges, so grout lines stay aligned with the rest of the field — this is why keeping a box or two of leftover tile from the original job matters.
